Health and Safety Inspections
What is a Health and Safety Inspection?
Most child care providers offer safe and healthy environments. Health and safety inspections and investigations are a normal part of child care in Idaho. Inspections are not intended to "catch" anyone doing something wrong. Inspections help ensure the safety of children and staff, support staff in following requirements by finding concerns, and promote continuous improvement.
Idaho began publishing the results of health and safety inspections October 2017. Inspections are published as they are conducted and maintained on this site for at least three years.

Incident Reports
What is a Substantiated Incident?
Occasionally, families and community members may report that a child care facility is not meeting requirements for safe and healthy child care. All child care incidents reported in Idaho are investigated. After an investigation, if it is determined by the relevant authorities that an incident occurred, it is considered “substantiated.” It is important to know that only substantiated incidents and allegations are published on this website. Incidents that are still under investigation will not be published until the investigation is concluded.
Idaho began publishing substantiated incidents in October 2017. Incidents are published as each investigation is concluded and maintained on this site for at least three years.
Child Abuse and Neglect
There were concerns of a staff member using excessive physical force with a child. At the time of investigation, it was confirmed that the staff member did use excessive force when handling a child.
Resolution
The facility self-reported the incident and the staff member was immediately terminated from employment. Law enforcement and the parents of the child were alerted. The staff member was issued a citation for injury to child.
Supervision
At time of investigation a child in the facility was found wandering to the middle floor of the facility where there were no staff members available to watch. A child was allowed to sleep on an elevated platform that was part of a climbing set. It was about 2-3 feet off the ground. Children were allowed to go to the bathroom without supervision. In addition, staff members were using hand sanitizer instead of washing hands after diaper changing. Other staff members were not using the 3 step method to clean diaper changing tables.
Resolution
Provider will retrain staff on diaper changing procedures to include how to sanitize the changing table using the three step method. Provider already has a site visit set up to observe staff changing diapers/sanitizing changing table/handwashing.
Provider will have staff not let children sleep on the loft in preschool classroom and put a sign on the loft letting staff know that children are not able to sleep on the loft.
When children need to use the bathroom one staff member will stand in the doorway of the classroom with one foot in the classroom and one foot in the hallway to ensure child does not wander upstairs.
Staff will be directly observing children at all times.

Facility was out of compliance with child-staff ratio, and a staff member left children unsupervised in a classroom.
Resolution
Provider created a plan for maintaining correct child-staff ratios, and adequate supervision. Provider shared plans with employees.
Pipes at facility are frozen. Kitchen and bathrooms are not operational.
Facility's restrooms and kitchen were not operational due to frozen pipes.
Resolution
Facility was closed for several days while plumber worked on pipes. Health Inspector visited and confirmed that water was running and kitchen and bathrooms were accessible and operational. Facility was reopened.
